Paper Forms & Notarization: When and Why They’re Required and How to Submit Them

Most ABLE account actions can be completed online through your account dashboard. However, some requests require paper forms.

Online Requests Are Recommended

When available, online tools are the fastest and most efficient way to manage your ABLE account. Online requests are submitted instantly, processed more quickly, and are easier to track. They also help reduce delays or errors that can occur when required information is missing or incomplete.

In the normal day‑to‑day, you can use your account portal to:

  • Update your email, phone number, and mailing address
  • Change your password or security settings
  • Set up or edit bank accounts 
  • Make one-time and repeated contributions or withdrawals
  • Send payments to third parties (bill pay)
  • Adjust your investment selections within the program’s rules

When Paper Forms Are Required

Some requests are higher‑risk, more complex, or legally sensitive. For those, we may require:

  • A signed paper form
  • A notary public stamp

Common examples where a form is often required include:

  • Adding, removing, or or changing an Authorized Legal Representative 
  • Changing the beneficiary
  • Post-death payments: Estate‑related payouts after death may require a form.
  • Rollovers: Moving funds between 529 or ABLE programs requires a specific rollover form.

Paper Form Processing Time

Paper forms generally take longer to process than online requests. Processing begins only after the fully completed form is received and reviewed. Mailing time, review time, or missing information may result in delays. Submitting complete and accurate forms helps minimize processing time.

Where to find the right form

You can find required forms on your ABLE program website (before you log in). Simply scroll to the bottom of the page and click on Forms under “Plan Resources.” 

What Types of Forms Require Notarization?

Forms that usually require notarization include:

  • Add an Authorized Legal Representative
  • Change of Authorized Legal Representative
  • Remove Authorized Legal Representative
  • 529 College Savings to ABLE Rollover (Direct)
  • 529 College Savings to ABLE Rollover (Indirect)
  • ABLE to ABLE Rollover
  • Bank Add/Change Request - (Required only if requesting exception of 10 day hold)
  • Manage Monthly Withdrawals (Required for withdrawals over $50,000 or to 3rd parties)
  • Death of Account Owner
  • Change of Name
  • Change of Address (Required only if requesting exception of 15 day hold)
  • Withdrawal (Required for withdrawals over $50,000 or to 3rd parties)

If a form needs notarization, it will be clearly stated on the form.

How to Get a Form Notarized

You can get a form notarized by:

  • Visiting a bank or credit union
  • Going to a shipping or office supply store
  • Using an online notary service (where available)

Be sure all individuals signing the form have a valid photo ID.

How to Submit Forms (and Supporting Documents)

Before you submit the form make sure to:

  1. Follow the instructions printed on the form.
  2. Include any requested supporting documents (for example, a death certificate, court order, or proof of legal authority).
  3. Keep a copy of everything you send, including any tracking or confirmation numbers.

Upload the form and required documents online. To do so:

  • Login to your account online.
  • Scroll to the bottom of the page and under “Questions?” click Contact Us
  • Click on Secure Document Upload and make your selections regarding the document. 
  • Upload the document and click Send.  
  • To upload multiple documents, simply repeat this process.

You can also submit forms and any required documents by mail, although this is not preferred and will result in longer processing times
